Obtain A Repeat Prescription?

Repeat prescriptions may be made by post (enclosing an SAE), in person at reception, via the pharmacy or by fax (01282 611958) or e-mail repeats.gp81757@nhs.net. Please give full details and your prescription will be ready to collect in two working days, excluding weekends/bank holidays.

REPEAT PRESCRIPTIONS CANNOT BE REQUESTED OVER THE TELEPHONE.

THIS FORM IS CURRENTLY DISABLED - PLEASE USE ONE OF THE ALTERNATIVE METHODS MENTIONED ABOVE TO REQUEST PRESCRIPTIONS.
